Personal Care & Social Connections

We work with seniors, adults and children, ranging from routine care to more complex medical needs. We will support you with various activities that are important to your social, mental and physical well-being. To avoid isolation and increase social interactions, we will provide you with a compassionate staff in the comfort of your home.  Your trust is important to us!
• Grooming, showering, dressing
• Feeding, meal planning and preparation
• Escorts (grocery shopping, doctor’s appointment, hearing health loss)
• Light housekeeping, social interactions (card games, reading, walking, etc.)

 

CARE PACKAGE 2

Personal Care & Social Connections Enhanced

We work with seniors, adults and children, ranging from routine care to more complex medical needs.
We will support you with various activities that are important to your social, mental and physical well-being. Our Personal Support Workers (PSWs) or Social Services Workers (SSWs) will continue to assist with your activities of daily living in your home, long-term care home or assisted living, in a safe and dignified manner. A Case Manager or Nurse will work with you or your loved one to create a care plan that is consistent and convenient for you. Your trust is important to us!
• Grooming, showering, dressing, feeding, medication reminders
• Assistance with feeding, meal planning preparation, escorts ( to and from activities of daily living)
• Light housekeeping and laundry
• Social interactions (card games, reading, walking, etc.)
• Exercise activities and mobility assistance (lifts, transfers)
• Medication reminders
• Alzheimer’s and Dementia Support
• Caregiver Relief & Respite Care

 

CARE PACKAGE 3

Personal Care Optimized

We will support you or your loved ones with a regulated professional nurse if you require clinical services as a result of illness, hospitalization, surgery or other conditions. We will continue to assist with activities that are important to your social, mental and physical well-being in a safe and dignified manner for as long as you require it.  Your trust is important to us!
• Grooming, showering, dressing, feeding, medication reminders
• Assistance with feeding, meal planning preparation, escorts ( to and from activities of daily living)

Vincam Habilitation

Helping those with special needs to live more independently

Vincam Habilitation is a program that helps clients with intellectual and developmental disabilities to live safely at home and participate in their communities with comfort, independence, and dignity. This specialty program serves clients across Ontario who have autism spectrum disorders, intellectual and developmental disabilities, cerebral palsy, and other developmental disabilities.

While many of the services we provide involve personal care assistance, what sets VINCAM apart is our focus on community integration. Our approach to habilitation involves teaching clients to do things for themselves — like brushing their teeth, managing money, shopping for personal items. Together, we help our clients to live more independently and safely at home and in the community, while offering periodic relief for family members and caregivers.

Professional Case Management Services

Our Case Managers are expert, compassionate regulated professionals, who work with clients and their families to deal with traumatic or challenging situations. These could be the result of a divorce, addictions, poverty, motor vehicle collisions and injury, discrimination, illness, and more. Our client-centered and multi-disciplinary way of working, means that regardless of how complex your needs are, we will coordinate with all the service providers to get you back on track or on the path to full recovery.

At Vincam Healthcare, our professional case managers have a diverse background and will be the liaison between you, the care providers or the insurance companies. The goal is to ensure all rehabilitation needs are met and all sources of funding are explored.

Some of our Case Management services include:

  • Conflict resolution
  • Crisis intervention
  • Discharge planning support with family/client
  • Adjustment to illness or injury
  • Assistance with access to community programs/ resources
  • Assess and coordinate the vocational, medical or rehabilitation needs for people injured in motor vehicle collisions
  • Assistance with access to short or long-term disability or workplace safety insurance benefits
  • Develop and coordinate return to work plans
  • Prepare independent examinations and medical legal reports (if needed)
  • Determine eligibility for funding support from federal and provincial programs, as well as extended health care, auto and workplace safety insurance

What to expect

At our first meeting, an assessment will be done to help us manage your treatment. We will  gather your health history, all reports, administrative and insurance details on your behalf. We will explore all funding options and determine which rehabilitation services you require and arrange access accordingly.

The Case Manager assigned to work with you can provide one-on-one service or include everyone involved in your care as part of the multi-disciplinary treatment team. Progress reports will be provided when needed; treatment plans will also be adapted when your progress or prognosis warrants it.
